An exercise bike is pictured surrounded by debris in the town of Pilger, Nebraska June 17, 2014. Residents who were forced to leave a Nebraska village leveled by a tornado that killed a child and injured more than two dozen people began returning on Tuesday to salvage belongings from their battered homes, businesses, a church and school. The town of Pilger took a direct hit Monday afternoon as tornadoes swept across a farming area in northeast Nebraska, devastating up to 75 percent of its buildings, officials said. Sandy Wright (L) receives some help getting into her exercise bike from her C.N.A. (Certified Nursing Assistant) Jessica Haynes at her home in Peoria, Illinois, November 25, 2013. Wright has Neuromyelitis Optica and has a C.N.A. to help her around the house three times a week. Now, patients like Wright are at the forefront of an experiment, under way in Peoria, Illinois, and hundreds of other U.S. cities, that could transform the way doctors, nurses and hospitals deliver care to patients. Amid the barrage of criticism over the rollout of Obamacare, groups known as Accountable Care Organizations (ACOs) are quietly going about the business of testing the potential for healthcare reform. The efforts, born of President Barack Obama's Affordable Care Act, are part of the biggest experiment yet to fix the costly and error-plagued U.S. healthcare system. The new models of care, which encourage providers to form networks to coordinate care and cut costs, involve close monitoring of the sickest patients to address budding health problems before they cause a costly trip to the emergency room or an extended hospital stay. Orty Gym owner Lynda Ellabou poses in her new all-women's gym on the outskirts of Paris, in Le Raincy, October 4, 2013. The gym has become the unlikely venue for a heated controversy in France over Muslim integration, secularism and what some charge is blatant populism in advance of upcoming municipal elections. The gym, which opened last month in the bourgeois suburb of Le Raincy, is owned by a French Muslim couple who say their religion and appearance - she wears a headscarf and he a long beard - are the reason the mayor wants to shut them down. Some of the 70 women who have already joined the room cover their hair with a headscarf, but many do not, as all races and religions are welcome, said Lynda Ellabou, who owns the gym with her husband. The wheels of an indoor bicycle are seen spinning at a SoulCycle class at their Union Square location in New York April 13, 2011. Indoor cyclists routinely sweat a real river pedaling down an imaginary road, but SoulCycle classes add upper body work, and a touch of the spiritual, to their metaphorical journey. Vu Cong Doanh, a 72-year-old Vietnamese man, exercises on the Long Bien bridge in Hanoi October 25, 2010. Vu Cong exercises twice a day on the bridge that for him represents the steel dragon giving strength and power. Long Bien bridge was built in 1903 with the architectural concept of the famous French architect Gustave Eiffel across the Red River to connect two parts of Hanoi. Bombed repeatedly during the Vietnam War because of it strategic value, the bridge was repeatedly repaired and now trains, mopeds, bicycles and pedestrian can use it to cross the river.
