HEALTH AND SAFETY EXECUTIVE ANNOUNCE TRAIN CRASH REPORT. 
Health and Safety Executive chairman Bill Callaghan (L) and Health and Safety Executive Chief Inspector of Railways Vic Coleman bow their heads for a minutes silence during a press conference to release the HSE's preliminary report October 8, on the train crash near London's Paddington Station. The report stated that the immediate cause of the accident appeared to be that the Thames Train passed a red "danger" signal some 700 metres before the collision point.
