Dr. Carlos Pellegrini, Department of Surgery chairman at the University
of Washington Medical Center (UWMC), shows how a surgical retractor is
used during a mock operation on a mannequin in Seattle, December 7,
2001. The UWMC disclosed December 7 that five incidents of surgeons
accidentally leaving instruments inside a patient post-operation have
occurred at the hospital since 1997. Most recently, Donald Church of
Lynnwood, Washington survived several weeks with a 13-inch long,
two-inch wide surgical retractor that was accidentally left in his body
